2008 SILVER CIRCLE NOMINATION FORM

The Silver Circle Award was established to recognize and honor outstandingindividuals who have devoted 25 years or more to the television industry and who have either made significant contributions to Chicago broadcasting, or who have spent the formative years of their career in Chicago. Nominees in academic, administrative, creative, journalistic, performing, or technical roles within the television industry are eligible.The information below, along with any additional materials supporting the nomination must be submitted to the Chicago/Midwest Chapter of the National Academy of Television Arts & Sciences for review and consideration by the Silver Circle Committee and the Board of Governors. Nominations remain active for three years from submission.
